Cornwall Partnership NHS Foundation Trust (CFT)

If you have a severe mental health condition, you may be referred to CFT Mental health and wellbeing for assessment and treatment. Your local Community Mental Health Team (CMHT) will be your first point of contact. Referral to the CMHT is usually via your GP, but you can also contact them directly. To find out which is your nearest team, please call 01208 251300.

Out of hours mental health services can be contacted on 0845 230 3901 for North Cornwall, Caradon and Restormel areas. 0845 230 3902 for Carrick, Kerrier and Penwith areas.

More information can be found on the Cornwall Partnership NHS Foundation Trust website.

If you are a carer you can also have an assessment of your needs, regardless of whether the person you look after receives support from us or not. This assessment will look at how your caring responsibilities affect you and what sort of help you may find useful. A personal budget may be available following your assessment. This is for you to purchase your own support.

The information you share with health and social care professionals will be treated in confidence. However, there may be some circumstances where the information needs to be shared with others. If you would like to find out more, please talk to the person working with you.

To find out about other support in your local area visit the Support in Cornwall website.

Visit your local library for self-help books on a variety of mental health problems. Find out more by visiting Reading Well Books on Prescription
